The template that names the plan, the term and the fees for one customer. Published so a buyer can read it before they are asked to sign one.

This Order Form is governed by the Momiji Subscription Agreement, version [n], effective [date], including the Data Processing Addendum and the Service Level Schedule (together, the "Agreement"), published at mymomiji.com/legal. Capitalized terms have the meanings in the Agreement. Where this Order Form and the Agreement conflict, this Order Form governs for this order.

Invoices may be issued by Greystone Strategic Partners Inc. as Momiji's billing agent. Payment terms: 15 days from invoice.
4. Special terms
Select those that apply; strike the rest.
- Charter customer. The plan rate above is fixed for the initial term and the first renewal. The launch service fee is waived. In return the Customer agrees to serve as a reference for up to two prospective customers per year and to participate in one written case study, the text of which the Customer approves before publication.
- Publicity. Momiji may / may not identify the Customer by name and logo in customer lists.
- Managed payroll. Momiji prepares and presents each run for the Customer's approval; the Customer remains the employer and remitter. Scope and cadence: [describe].
- Multi-organization. The Customer operates the organizations listed in Annex A under this Order Form; usage is measured across them.

5. Acceptance
By signing, each party agrees to this Order Form and the Agreement.
